CentacareCQ has an opportunity for a full-time Support Facilitator, Partners in Recovery (PIR) to join their dedicated team based in Rockhampton.
The successful applicant will be responsible for developing and delivering collaborative partnerships, and assisting people affected by severe and persistent mental illness to access appropriate support services. The Support Facilitator will also be responsible for identifying gaps in services for their clients and making recommendations on methods by which these gaps can be addressed.
Duties of the role include:
Facilitate coordination of clinical and other supports and services to deliver person centred care tailored to individual needs
Network with relevant internal and external stakeholders to ensure appropriate referral for individuals
Network and develop collaborative relationships with relevant community service and government organisations to successfully link clients to relevant support services
Maintain referral pathways that facilitate access to a range of services and supports needed by the target group
Prepare and compile regular statistical and informational reports by effectively using the PIR data management and client information systems
Participate in support Facilitator meetings
Support the transition of PIR clients to NDIS
Manage brokerage funds associated with the program
